Monitoring oxygenation only, which is a standard practice for monitoring patients on noninvasive ventilation, may be quite inadequate, particularly for patients with acute respiratory failure. Ventilation is frequently a significant issue with these patients, and having the ability to monitor ventilation with the use of capnography may result in improved care and patient safety.

Waveform capnography represents the … Background: The Anesthesia Patient Safety Foundation has advocated the use of continuous electronic monitoring of oxygenation and ventilation to preemptively identify opioid-induced respiratory depression. In adults, capnography is the gold standard in respiratory monitoring. An alternative technique used in sleep laboratories is respiratory inductance plethysmography (RIP). Using parameters such as oxygenation, ventilation, circulation, temperature, and response to stimulation is integral to patient monitoring. 1 Guidelines are often implemented to improve patient safety, reduce risks, and standardize practice.

Pulse oximetry provides instantaneous feedback about oxygenation (see "Pulse oximetry"). Capnography is useful in patients receiving supplemental oxygen. In these patients, oxygen desaturation reported by pulse oximetry may lag during episodes of respiratory depression and apnea. Capnography indicates partial pressure of end-tidal carbon dioxide and provides information regarding airway integrity and patterns of ventilation compromise. Capnography can measure ventilation by measuring end-tidal carbon dioxide (EtCO2). However, it does not provide a measure of any form of oxygenation. Conversely, pulse oximetry measures arterial oxygen saturation but is unable to provide a measure of alveolar hypoventilation or hyperventilation.

The American Society of Anesthesiologists (ASA) emphasized the importance of using capnography monitoring outside of the operating room (Kodali, 2013).
